Mckenzie Method works great for my neck - used many, many times.
Mckenzie Method is working great for my back, when I finally was really careful to use most of its many techniques and advice points pretty much properly.
Also, I believe being more properly in line with my Heavenly Father's plan, is probably the most important aspect to my recovery of all.
In addition, rest, heat, temporary avoidance of bending, lifting; also continued physical activity gradually increasing, improved posture, limited sitting, standing, lumbar supports, proper posture in getting into and out of bed and when lifting, etc (God, Mckenzie, Posture, Activity and ...)

Gokhale Method and TENS have been the most helpful along with mindfulness meditations. Gokhale because it is a comprehensive focus on lifestyle, posture for sitting, standing, lying and moving (walking) and provides exercises to develop strength and flexibility. It also teaches me more of the physiology involved. It allows me to take control of the pain issues rather than be controlled. I work on it every day and maintain a status as an alumni to be able to attend webinars, ask questions and access the video library. TENS is good when the back is stiff and sore and provides immediate relief.

One medication I am on that is not on your list is Gabapentin
